Describe the bug
File upload does not work properly in new request

To Reproduce
Steps to reproduce the behavior:

  1. Go to 'new request'
  2. Click on 'upload file'
  3. Click 'submit'
  4. See error

Expected behavior
File should appear in the view request page after it has been uploaded
